Regenerative Medicine

Autologous (refers to cells, tissues or even growth factors that are re-implanted in the same individual as they come from.) In other words the patient receives his or her own cells. Cell therapy injections are a by-product of blood that is rich in growth factors and platelets. Until now, it’s use has been confined to the hospital setting. This was due mainly to the cost of separating, the cells from the blood and the large amount of blood needed to produce a suitable quantity of cells from only 30 to 50 cc of blood drawn from the patient while they are in the office setting. It takes about 20 minutes to harvest these cells prior to the injection.
During the healing process, the body rushes many cells and cell types to the injected area in order to accelerate the healing process. These cells perform many functions, including the release of growth factors (GF) into the diseased or arthritic and injured sites. Another main function is attracting Mesenchymal Stem Cells to the damaged areas. These growth factors (Vascular Endothelial Growth Factor (VEGF1) function to assist the body in repairing itself by stimulating stem cells to regenerate new tissue. The more growth factors released sequestered into damaged cells, the more stem cells stimulated to produce new host tissue. Thus, one can easily see that Cell Therapy and its applications for injection into the joints and tendons to improve osteoarthritis and sports injuries. Currently Cell Therapy has been used for pain and arthritis of Knee, Shoulder, Spine, Hip, Ankle, Wrist and Elbow at The Center For Regenerative Medicine.
A subfamily of these cells, are Mesenchymal Stem cells (different from the embryonic stem cells). These stem cells are contained within each individual and are derived from the person’s self. They have been shown to be multi potent, and have the potential of becoming, cardiac cells, nerve cells, Hepatocytes (liver cells), Chondrocytes (cartilage cells), and even vascular cells (angiogenesis). Angiogenesis is of great significance to the Orthopedic Regenerative physician whose goal is the improvement of arthritis and sports injuries.

Degenerative Joint Disease (DJD) or more commonly known as Arthritis (In Latin ‘arth’ means joint, "itus" means inflammation) is not an isolated problem as it is becoming an epidemic as the aging population lives longer; it actually consists of more than 100 different conditions. These can be bursitis to crippling systemic, forms such as rheumatoid arthritis.
Do these treatments help only arthritis?
The treatments can help most Muscular skeleton problems
such as neck pain, back pain, shoulder pain, wrist pain, knee pain, hip pain, foot pain, sciatica, tendinitis, Tennis elbow, sprains, strains, whiplash and torn ligaments and cartilage.
How many treatments does a person need? Is there any down time?
The number of treatments depends on the tissue damage, severity of the condition, the size of area that need to be injected and most importantly the patient’s healing capability. People usually need a series of about 1 - 6 treatments to improve. Each treatment is independent of all the others, meaning each treatment by itself has benefits, there is usually no down time and people can go back to work or their usual activities immediately.
Are any of the medications used FDA approved?
All the medications used are FDA approved. The use of the medications for injection into ligaments, tendons and joints is an "off-label" use. This means that an FDA approved medicine can be used for other than the FDA approved uses if the physician believes the medication is beneficial for a patient.
In some cases Platelet Rich Plasma is used.
What is your patient demographic?
Since the project LaJRaN was started, the center has done over 10,000 procedures. Baby boomers comprise of 40% of the patients, 40% are 61-years and older and about 20% are in the 13-40-year-old group.

Knees are by far the most common problematic areas in the practice, followed by the shoulders and spine (back and neck).
75% of patients are Americans, 20% are from South American countries, and the rest come from different countries as far as Hong Kong, South Africa, and Australia.
Younger patients whom also are the fastest growing population of patients seen at the center are mostly professionals, including the NFL, NBA and PGA, Baseball players, Tennis Players, Dancers, Politicians, Doctors, Lawyers, and Law Enforcement.
What is the most important goal in this procedure?
Again, it depends on each patient, some want to just be pain free, some are looking for range of motion, some need more strength, some want to go back to activities that brought the this far such as playing Golf, Tennis, Dance, Soccer and Football.
For others it is important not to think twice about getting up to get a glass of water or just being able to walk again.
In general people like to be functional again.
What is your advice to people out there?
Do not abuse your muscular skeleton system, it is very fragile, it doesn’t take much to injure, it is easily damaged, and it is not easy to fix it.
Lose weight, 70% of Americans are overweight. Your weight bearing joints, meaning your ankles, knees and hips are not made to carry any fore than 20% more than your ideal body weight, this is why 60% of Americans over the age of 60 have arthritis in their knees, 60% is a big number. Cautiously take some supplements, some of the medications available actually can help.
About the Doctor:
Alimorad Farshchian MD (farsheean) was conferred his Doctorate of Medicine (M.D. Degree) with High Honors from Spartan Health Sciences University School Medicine in 1987. He then joined the highly acclaimed research staff at State University of NY (SUNY Down State), a subsidiary of New York Research Foundation. He pursued further education with an Internship at the University of Tennessee’s College of Medicine.
